Consent

By using the online booking system, completing an initial consultation form or by supplying your information during consultation, you are giving your consent for Ellie Guest Massage Therapy to hold your personal information.

Information provided verbally, electronically or written on forms is held with your clinical records. Your information is held in order to maintain records of treatment and progression. I use your email address to send newsletters with relevant service information, You can unsubscribe from the newsletter at any time by clicking the unsubscribe link at the bottom of the email.

Storing Your Information

I store you information securely at all times. Your name, email address, phone number and appointment history are stored securely in my 10to8 online booking system. Additionally, your email address is held in a Mailchimp mailing list. Paper documentation is never removed from the studio. It is stored in a locked cupboard in the studio, accessible only by me.

Retaining Your Information

You have the right to ask me to destroy some or all your records at any time if you wish to terminate our working relationship. Your records are destroyed eight years after your last contact with me.

Accessing Your Information

I am the only person who can access your records. You have the right to request a copy of your records at any time. Once requested, I will send them in PDF format attached to an email within seven working days.

Sharing Your Information

Your information is kept strictly confidential. I do not disclose your personal and treatment details to anyone else.

Complaints

If, for any reason, you are unhappy with how I am handling your data, please raise your concerns with me first, so that we can seek a resolution. If you are still not satisfied, then you have the right to complain to the Information Commissioner’s Office.
